We welcome this opportunity to explain some of the membership benefits and requirements for joining
the Yakima County Medical Society.
Requirements:
- A degree of Doctor of Medicine or Doctor of Osteopathic Medicine.
- License from the State of Washington to practice medicine and surgery, or osteopathy and surgery.
- Maintain a practice in Yakima County.
- As a "Unified County", membership in the Washington State Medical Association or the Washington
Osteopathic Medical Association is required.

Instructions:
An Application Form and Release from Liability Form follow these instructions. Either
print out copies
to complete or call the YCMS office at 575-5550 and request copies to be mailed.
Two members of YCMS who know you should sign the Application Form and also submit letters of
recommendation on your behalf.
A $100 check for Applicant fees should accompany your application.
These fees entitle you to membership dinners and eligibility for group health insurance, as well
as all other benefits accrued to Active members, with the exception of voting and holding office.
With Board of Trustees approval of your candidacy, you will be eligible, by vote of the Society,
for full Active membership. Regular County/State dues are not assessed until you become an Active
member.
Please enclose two 4"x5" photographs (or a digital picture on a disc) for inclusion in our website
and printed Membership Directories.
